Please provide:

• Rare disease/condition name and your Medical Specialist's (not General Practitioner or Therapist) email address

• Proof of five year Australian Residency/Citizenship (e.g. Passport or Birth Certificate) 

• Coloured copy of Medicare Card

• Private Health Insurance details

• Government assistance details e.g. National Disability Insurance Scheme (NDIS) Plan and supporting evidence of NDIS approval or decline.

• Quote/Invoice for items or services being requested

• Income and expense details

• Tax return/payslip
